New International Travel Request & Waiver
Individuals planning to travel internationally now have a simplified request process. The previous Faculty/Staff International Travel Request Form and the International Travel Waiver Form have recently been combined and renamed the International Travel Request & Waiver Form. The form now also accommodates students requesting to travel internationally (non-study abroad) and is available for easy access and signing on DocuSign.
Faculty, staff and students should consult the Policy on Travel Advisories for International Travel and Travel to a U.S. Territory for specific details before participating in any international travel programs or activities. Travelers should also consult the Business Travel Office website for information on international travel policies, allowable costs, travel arrangements, and payment options.
