Last Day for Open Enrollment in Optional Life and Long Term Term Disability Coverage
Don’t miss this limited time offer to increase your Optional Life and Long Term Disability (LTD) coverage with reduced rates through USM MetLife and simplified enrollment process on the GIS / Boon-Chapman platform.
USM has contracted with MetLife as the Life Insurance and Long Term Disability (LTD) Insurance carrier for employees and their covered dependents.
These benefits will now be administered on the GIS / Boon-Chapman platform and are separate from the benefits offered through the State of Maryland.
Please note that this is a separate benefit from the State of Maryland. If you previously had UNUM your plan will automatically be converted.
To enroll or make changes:
- Log in using your W-number and the initial PIN/password is the last four digits of your W-number + the four digits of your birth year. (If you don’t know your W-number, reach out to your Benefits Coordinator.)
- Answer just five health questions to apply for Optional Life coverage.
- You can also enroll in the LTD plan (365 day elimination period option) without having to answer any health questions – this normally requires you to provide medical information. Choose the coverage level right for you from a range of cost-effective options.
Any elections you make during this enrollment period will go into effect on May 1, 2021 or the first of the month following MetLife’s approval of your request.
