To provide clarity about Family and Medical Leave and improve leave processing, the Office of Human Resources (OHR) updated the Request for Family Medical Leave Form.  Please discontinue using the prior version of this form.

This form is located on the OHR web page at – https://www.towson.edu/hr/current/leave.html, along with the medical certification form required with the request for Family Medical Leave.
